Vaccination Card

If possible, upload a copy of your dog’s up-to-date vaccination card or clinical history, which should include kennel cough, distemper, canine hepatitis, parvovirus, and leptospirosis. The vaccination must have been administered at least two weeks before boarding. The upload must also include your name, address, and the name & microchip number of your dog.
